If you are new to the practice we suggest you download and complete our New Patient Registration Form from our Downloads Page and bring it with you on the day of your appointment.

If you decide to complete this on the day of your visit we suggest you arrive 10 minutes prior to your appointment to allow time to complete your registration.

• We recommend all new patients bring a list of their current medications to their first visit.
• You will also need your Medicare card and any pension, concession or DVA cards.
• Pensioners, Health Care Card holders and children under 16 will be bulk billed.

If you do not have a Medicare card the Private fee applies and payment is required on the day.
